Information we collect
When you submit an inquiry, Ageility may collect your name, email address, optional phone number, the type of inquiry, the message you provide, and the date and time of submission. Basic technical information may also be processed by the hosting provider to deliver and protect the website.
How information is used
Inquiry information is used to respond to you, understand the nature of your request, determine whether Ageility may be an appropriate fit, coordinate a referral or partnership conversation, and maintain reasonable business records. Ageility does not sell personal information.
Please do not submit sensitive health information
The website inquiry form is intended only for general introductory information. Do not submit medical records, insurance information, government identifiers, detailed diagnoses, payment information, or emergency health concerns. If more detailed information is appropriate, Ageility will identify a suitable communication method after the initial conversation.
Storage, protection, and retention
Website inquiries are stored through Ageility’s hosting infrastructure and are available only through a restricted administrative view. When email notifications are enabled, the inquiry is also transmitted through Ageility’s email-delivery provider so an authorized administrator can respond promptly. Reasonable administrative and technical safeguards are used, but no internet transmission or storage system can be guaranteed to be completely secure. Information is retained only as long as reasonably necessary for the inquiry, the relationship, applicable recordkeeping, and legitimate business purposes.
Spam and rate protection
To protect the inquiry form from automated abuse, the website may use a short-lived cryptographic identifier derived from connection information. Ageility’s application does not store the underlying network address in the inquiry record, and the rate-protection records are automatically limited to a short operational period.
Service providers and required disclosures
Ageility may use service providers that support website hosting, security, communications, or operations. Information may also be disclosed when required by law, to protect legal rights or safety, or in connection with a legitimate organizational transaction subject to appropriate protections.
Your choices
You may ask Ageility to update or delete inquiry information, subject to legal and legitimate recordkeeping requirements, by contacting Jeremy@Ageility.org. You may also choose to contact Ageility by telephone instead of using the website form.
